Hospice care is delivered by an interdisciplinary team to patients who are no longer appropriate for aggressive curative treatment. Hospice focuses on quality of life, is patient-centered and takes place at home or in a skilled nursing facility. Our team is comprised of a board-certified physician, a nurse case manager, clinical social worker and pastoral support. A registered nurse is on-call 24/7 to help answer any questions you have and/or make a visit if the need arises.
